WITHDRAWAL OF CHILD FROM CARE

Notice of termination is required.  A two week notice must be provided in writing in advance indicating your intent to withdraw your child/children from care.   If two weeks notice is not given you will be charged for the two weeks and forfeit your deposit.

 

GUIDELINES FOR RELEASING CHILDREN

Provider will release child only:

1. To parents with legal custody or the child’s legal guardian, or

2. To anyone the custodial parent has authorized by prior arrangement with provider in writing, or

3. To police or welfare workers with proper ID

Anyone removing the child other than the parent or the child’s legal guardian must show proper identification to the provider before the child will be released. ___________(Parent/Guardian initials)

 

CARE FOR EMERGENCIES

If child has to be transported to hospital I will provide a substitute for other children. 

When I am on vacation or daycare is not open I will not have a substitute and you will need to secure care for your child. _____________ (Parent/Guardian initials)

 

MODIFICATIONS/AMENDMENTS

The Provider reserves the right to modify and or amend this agreement upon two weeks written notice in the basic rates or services. ____________(Parent/Guardian initials)
