CHILD ILLNESS POLICY

I will notify you immediately should your child develop any of the following symptoms:

Temperature over 100 degrees

Vomiting or diarrhea

Should your child develop any of these symptoms, you will be expected to pick up your child within one hour.  If this is not possible, you will need to have another person listed on your emergency information form that can.

I not take a child with confirmed lice unless the hair has been washed with an approved product twice (24 hours apart).  I will check the child’s head personally upon arrival.  Should I find anything, your child will not be allowed to stay. ______________(Parent/Guardian initials)

 

TRIAL PERIOD

The first two weeks of the contract is a trial period.  During this period, the developmental or special needs of the child will be assessed.  In the event the needs of the child will not be met by the provider’s program, either the parent or the provider may terminate this contract without further notice or fee.  Any fees paid during the trial period will not be refundable. _____________ (Parent/Guardian initials)

 

CANCELLATION BY PROVIDER

Immediate cancellation:

The provider may terminate the child’s enrollment in the provider’s program effective immediately upon written notice to parent if any of the following conditions are met:

  1. In the sole judgment of the provider, the child’s behavior threatens the physical or mental health or well-being of one or more of the other children in the daycare.

  2. Any payment due has not been paid within one week after due date

  3. Child is picked up late more than three times in any thirty day period.

Two week notice:

The provider may terminate child/children’s enrollment in the daycare effective upon two weeks written notice if any of the following conditions arise:

  1. Any of the conditions listed under immediate cancellation are met

  2. In the sole judgement of the provider, the provider’s program does not meet the developmental or special needs of the child children.

  3. Parent/Guardian fails to abide by any other terms of this agreement; or

  4. The provider ceases to operate the daycare
